The Summit County Clerk of Courts office accepts passport applications from first-time applicants and renewals for passport holders that are fifteen and under at all three of its Title Division locations.
Passport applications will be accepted by appointment only from 8:00 a.m. to 4:30 p.m. on Monday through Friday (except for Legal Holidays), 8:00 a.m. to 12:45 p.m. on Saturday at the Main Branch, and 8:00 a.m. to 11:45 a.m. on Saturday at the Northfield Branch. Please call 330-630-7200 to schedule your appointment. We strongly recommend printing and completing the appropriate forms from travel.state.gov prior to the appointment time. For those without computer/printer access, forms are available in the branch lobbies but must be completed prior to the assigned appointment time.
Applying in Person
You should apply in person if:
- You are applying for your first U.S. passport
- Your most recent U.S. passport was lost, stolen or damaged
- Your most recent U.S. passport was issued more than 15 years ago
- Your most recent U.S. passport was issued when you were under age 16

Provide a Filled but Unsigned Application for Passport, Form DS-11
Do NOT sign the DS-11 application form until the Passport Acceptance Agent instructs you to do so. You can obtain the application form in a number of ways, including:
- Download Form DS-11 online
- Get form in person at a Title Division branch
- Call (330) 630-7200 to have forms mailed to you
NOTE: All minors 15 and under must appear in person, with consent from both parents or legal guardians. Learn more on the state travel website.

Present Proof of U.S. Citizenship
You may prove U.S. Citizenship with a previous U.S. Passport or a certified birth certificate issued by the city, county or state.
- Passports that are mutilated, altered, or damaged are not acceptable as evidence of U.S. citizenship.
- A certified birth certificate has a registrar's raised, embossed, impressed or multicolored seal, registrar's signature and the date the certificate was filed with the registrar's office, which must be within 1 year of your birth.
- All documentation submitted as evidence of U.S. Citizenship will be returned in a separate mailing from your new passport.

Present Proof of Identity
You may prove your identity with any one of these, if you are recognizable:
- Previous U.S. passport (mutilated, altered, or damaged passports are not acceptable as proof of identity.)
- Naturalization Certificate
- Current, valid driver's license
- Current, valid government ID: city, state or federal
- Current, valid military ID: military and dependents

Provide One Passport Photo
Your photograph must be:
- 2x2 inches in size, full color
- Showing current appearance, within last 6 months
- Full face, front view with a plain white or off-white background
- Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
- Taken in normal street attire

Pay the Applicable Fees

Provide a Social Security Number
If you do not provide your Social Security Number, the Internal Revenue Service may impose a $500 penalty. If you have any questions please call your nearest IRS office.
Expedited Service
Cost in addition to regular application fees, is $60 per application plus overnight delivery. Two-way overnight delivery is strongly suggested
If you mail in - clearly mark Expedited on the envelope
Anyone may request expedited service for any type of application (e.g., first-time applications, renewals, amendments of existing passports, etc)
Passports will be mailed separately. Passport applications sent together or at the same time do not necessarily remain together.
